OPERATING EXCAVATOR WITH MISSING BOOM

INCIDENT 

The arm structure of a Hitachi ZX360LC-5A excavator was removed to undertake repairs. While this was disconnected the upper structure was rotated. After turning about 90°the excavator tipped over to the point where counterweight came into contact with the ground



RECOMMENDATIONS 

The risk of falling is increased in model with heavier counterweights, however to minimise this risk all counterweights must be removed before turning without the boom attached.

 

This incident also highlights the importance of the Key Safety Promise: Never approach operating equipment without contacting the operator.
